Occupational Therapist (Per Diem) – Home Safety & Accessibility Evaluations
At 7TH & Madison Extensions of Empowerment, we’re expanding our statewide network of licensed Occupational Therapists (OTs) to deliver Assistive Technology (AT) and Home Modification (EM) evaluations, training, and follow-up support as needed.
This is a per diem, flexible position with in-home fieldwork available across all 21 New Jersey counties — including Atlantic, Bergen, Burlington, Camden, Cape May, Cumberland, Essex, Gloucester, Hudson, Hunterdon, Mercer, Middlesex, Monmouth, Morris, Ocean, Passaic, Salem, Somerset, Sussex, Union, and Warren.
Position Highlights

Key Responsibilities
- Conduct in-home Assistive Technology and Home Modification evaluations
- Assess functional needs and recommend adaptive equipment, AT devices, and home safety solutions
- Complete detailed, compliant evaluation reports aligned with State of New Jersey standards
- Provide member, family, and staff education on evaluations and recommendations
- Fit, adapt, and train members on AT devices when required
- Maintain professional and ethical standards in all interactions
- Include product photos and accessible links for recommended AT, DME, or EM solutions within reports to support clarity and implementation

Qualifications
- Active New Jersey Occupational Therapy license (required)
- Experience working with adults with developmental or intellectual disabilities (ID/DD) (required)
- Background in Assistive Technology and accessibility evaluations (required)
- Strong written communication and documentation skills
- Collaborative, compassionate, and member-centered approach
- Reliable transportation and valid New Jersey driver’s license
Experience:
- Assistive Technology Evaluations – 1 year (Required)
- Working with adult populations – 2 years (Preferred)
- Working with individuals with developmental disabilities – 1 year (Required)
License/Certification:
- Active New Jersey Occupational Therapy License (Required)
