Tiffany T. Weems, Founder & CEO
Tiffany founded 7TH & Madison Extensions of Empowerment (7TH & Madison) in July of 2011.
After graduating from Rutgers, the State University of New Jersey with a Bachelor of Arts Degree from the School of Public Affairs and Administration, majoring in Public & Non-Profit Administration, and minoring in Psychology, Tiffany identified her passion in May of 2015 and founded the first Division of Developmental Disabilities (DDD) approved Support Coordination Agency in Newark, New Jersey. Her first position with 7TH & Madison, was as a Support Coordinator Supervisor, providing oversight of all Support Coordinators within the organization, ensuring the organizations adherence to all Division of Developmental Disability policies and procedures as outlined by the Department of Human Services. While utilizing national best practices to administer social services that assist members in gaining access to needed program and State plan services, as well as needed medical, social, educational, and other services. Tiffany was responsible for ensuring the Individualized Service Plan was accurately developed and completed with the participant, their family, the Support Coordinator, and other team members designated by the participant. Tiffany was also responsible for verifying and reviewing ongoing monitoring of the provision of services included in the Individualized Service Plan. Tiffany also developed and conducted employee and member trainings as well as mentored at-risk youth within her community.
As 7TH & Madison grew, so did Tiffany’s responsibilities. In 2017, Tiffany administered all aspects of 7TH & Madison programs supporting people with developmental disabilities, at-risk youth, and seniors. She oversaw staff development and training and continued to maintain compliance with agency policies and contractual obligations. Tiffany also oversaw all agency quality assurance functions. Tiffany is responsible for opening a day habilitation program (day program) in Essex County, New Jersey for adults with developmental disabilities. Tiffany has also developed a Division for Mental Health Services within 7TH & Madison, which includes Intensive In Community (IIC) services, a partnership providing specialized tutoring, mentoring, with Partnership for Essex, the Care Management Organization (CMO) for Essex County, career planning, prevocational training, supported employment, respite, community-based supports, summer camp, along with additional supportive services. Tiffany successfully achieved targeted levels of services, facilitated and secured opportunities for growth and development. During this time, 7TH & Madison supportive service programs expanded from 3 to 12.
In 2019, while administering all aspects of 7TH & Madison programs which includes supporting seniors, at-risk youth, and individuals with developmental disabilities; Tiffany facilitated an agreement with NJ Transit, Access Link Division to administer assessments to participants. During this time, 7TH & Madison supportive service programs expanded from 12 to 14.
Tiffany adhered to nationwide best practices to administer social services with full-time and part-time employees and managing budgets of more than $4.5 million annually. Focused leader with proven ability to transform, improve, and optimize global, digitally enabled support operations across diverse verticals; experienced in leading support, success, professional services, premium, renewal and member focused driven teams. Lead direct reports to manage 80 cases statewide in account resolution, info management and governance, and budget modernization for over 5.5M annually. Oversaw 23 customer care staff, 2 outsourced engineers, $10.5M+ budget across state-wide support centers to deliver exceptional support function; created and led customer-driven programs. Recruited, trained, and led a new 25-person member success team from ground up to drive 60% of company's total flagship product line revenue, while ensuring efficient, and effective member support. Tiffany developed and implemented short and long-range plans and continued to oversee the expansion of programs, including developing new programs and activities. She developed and operationalized 7TH & Madison statewide Support Coordination program and continued developing and maintaining close working relationships with members, families, legislative leaders, and community organizations.
