Tiffany T. Weems, Founder & CEO
Tiffany founded 7TH & Madison Extensions of Empowerment (7TH & Madison) in July 2010.
After graduating from Rutgers, The State University of New Jersey, with a Bachelor of Arts in Public & Nonprofit Administration and a minor in Psychology, Tiffany identified her passion for creating sustainable, community-based solutions for individuals with developmental disabilities.
In May 2015, she founded the first Division of Developmental Disabilities (DDD)-approved Support Coordination Agency in Newark, New Jersey, becoming one of the early pioneers of New Jersey’s transition to the Fee-for-Service model. During this time, Tiffany also established a Durable Medical Equipment (DME) division and formalized a partnership with Aetna, connecting members to adaptive equipment and accessibility resources that improved daily living. This partnership laid the foundation for 7TH & Madison’s long-term commitment to Assistive Technology (AT) and inclusive accessibility solutions.
Her first role within the organization was Support Coordinator Supervisor, overseeing all coordinators and ensuring adherence to DDD and Department of Human Services policies. Tiffany developed Individualized Service Plans (ISPs) in collaboration with participants, families, and multidisciplinary teams, ensuring each plan aligned with person-centered goals and state requirements. She also created employee and member training programs and mentored at-risk youth in her community — demonstrating her passion for leadership and advocacy early on.
As 7TH & Madison grew, so did Tiffany’s leadership. By 2017, she was administering all programs supporting individuals with developmental disabilities, seniors, and at-risk youth. She oversaw staff development, compliance, and quality assurance functions, and successfully launched a day habilitation program for adults with developmental disabilities in Essex County. Tiffany later expanded the agency’s reach by creating a Division for Mental Health Services, offering Intensive In-Community (IIC) services, specialized tutoring, mentoring, and career planning in partnership with Partnership for Children of Essex (the Care Management Organization for Essex County). Under her leadership, 7TH & Madison’s programs expanded from 3 to 12 statewide services.
In 2019, Tiffany facilitated an agreement with NJ Transit’s Access Link Division to administer accessibility assessments to participants. During this time, 7TH & Madison’s programs expanded from 12 to 14, incorporating Durable Medical Equipment coordination and Assistive Technology integration into its service model to better support accessibility, independence, and safety for members statewide.
With more than 15 years of experience in the disability and human services field, Tiffany has managed multimillion-dollar budgets, built statewide teams, and led programs that combine compassion, innovation, and operational excellence. Her strategic leadership continues to guide 7TH & Madison’s evolution from a traditional social service agency to a statewide leader in Assistive Technology, Environmental Modifications, and Accessibility Services — empowering adults and older adults to live independently and thrive in their communities.
