7TH & Madison Extensions of Empowerment is a women-founded Assistive Technology organization based in New Jersey, dedicated to advancing accessibility, safety, and independence for adults with intellectual and developmental disabilities (ID/DD) and older adults aging in place.


With over 15 years of experience in disability and human services — including Support Coordination, Care Coordination, and community-based program development — our team brings a deep understanding of how to connect individuals to the right technology, resources, and supports that improve quality of life.


Today, we specialize in Assistive Technology (AT) and Environmental Modification (EM) evaluations, adaptive equipment recommendations, and training that help individuals live more safely and independently within their homes and communities.


Our approach is personal, compassionate, and outcomes-driven — every member is known by name, respected as an individual, and supported through solutions tailored to their unique needs.


Recognized for our white-glove service standard, we proudly serve adults and older adults across more than 21 New Jersey counties, collaborating with Managed Care Organizations (MCOs), state agencies, and community partners to deliver life-changing accessibility solutions.

About Our Founder

Our Founder and Chief Executive Officer, Tiffany T. Weems ensures all employees share a common drive and passion when delivering our services. She was born and raised in Newark, New Jersey and has more than two decades of experience in the Horizon Blue Cross Blue Shield systems.

Our Founder & CEO has more than 17 years of comprehensive experience at leading multi-county, state-wide operations with emphasis on equity and inclusion. Experience in residential development, case management services, Intensive In-Community Services (IIC), job coaching, career planning, community-based supports/services, along with over 18 federally funded services; that assist our members in gaining access to needed program and State Plan Services; as well as needed medical, social, educational, and other services. Focused leader with proven ability to transform, improve, and optimize global, digitally enabled support operations across diverse verticals; experienced in leading support, success, professional services, premium, renewal and member focused driven teams. Lead direct reports to manage over 500 cases statewide in account resolution, info management and governance, and budget modernization for over 5.5M annually. Oversee and lead 40 customer care staff, 2 outsourced engineers, and $5.5M budget across global support centers to deliver exceptional support function; created and led customer-driven programs. Recruited, trained, and led a new 25-person member success team from ground up to drive 60% of company's total flagship product line revenue, while ensuring efficient, and effective member support. Responsible for ensuring the Individualized Service Plan was accurately developed and completed with the participant, their family, the support coordinator, and other team members designated by the participant. Also responsible for verifying and reviewing ongoing monitoring of the provision of services included in Individualized Service Plans.

Industry Background

The founder began her health insurance career with Horizon Blue Cross Blue Shield in New Jersey. She served as a Suspense Quality Analyst while investigating and resolving Executive-Level Inquiries.

Our founder works with a number of charitable community organizations in the state of New Jersey. One of them is president at an organization that works directly with the American Cancer Society. She also serves as a mentor to individuals that are developmentally disabled by providing them with internships and employment opportunities.

The Individualized Service Plan (ISP)

Our staff applies a person-centered process to identify goals, provide support, and achieve results. We do this by developing and maintaining an Individualized Service Plan (ISP) that meets our Members' needs and preferences. The program is monitored by a Support Coordinator.

Behavioral Assistance Services

This program is a process designed to target and modify specific behaviors that interfere with a child's or youth’s functioning. It includes outcome-oriented interventions and ongoing evaluations designed to supplement the behavioral plans of care prepared by Licensed Healthcare Practitioners.
