Job description
The Quality Coordinator (QC) leads the operations of a team by providing administrative oversight concerning direction, strategic planning, quality assurance and supporting the coordination of service delivery for those employees under their supervision. They oversee the overall operation of Team functions, staff performance and programmatic implementation including responsibility for planning, assigning, evaluating, and reviewing the work of their staff. They provide input in the coordination and improvement of services and work with provider agencies to ensure timely and efficient service delivery. In addition, the Quality Coordinator may be asked to serve on task forces/committees and represent 7TH & Madison Extensions of Empowerment to the general public. Concurrently, they interact with other agencies, provide information to, and respond to requests from individuals and their families, community representatives, Providers and the general public which involves explaining programs and policies, handling complaints and grievances and speaking before the public and groups.
The Quality Coordinator reviews the Individualized Service Plans (ISPs) and Person-Centered Planning Tools (PCPTs), that are created by the Support Coordinators (SCs) and the individuals they support to verify that the plans represent best practices, meet state standards, and satisfy the needs of the individual. Reviewing these plans as well as tracking outcomes against numerous metrics on the ISP scorecards for each SC allows 7TH & Madison to proactively determine which Support Coordinators are doing an exceptional job and which Support Coordinators need additional mentoring. As a result of these reviews, 7TH & Madison identifies specific areas that will be enhanced through additional training for the support coordinators to improve quality of service to the individuals.
Job Duties and Responsibilities:
- Mentors assigned Support Coordinators.
- Establishes goals, timetables, and conducts monthly supervisions/annual evaluations of goal achievement for each Support Coordinators. Supports and encourages staff’s goals and objectives.
- Provides input to Directors in the preparation/analyses of their offices’ respective annual financial operating budget.
- Assesses monthly performance of their Team in relation to and advises as needed. Completes supervision forms on staff monthly for uploading into 7TH & Madison internal database.
- Recommends and implements corrective action as necessary.
- Monitors Team to ensure timely implementation of company policies and procedures.
- Monitors Team to ensure staff meet all requirements of local, state, and federal regulations and laws relating to the provision of waiver- based services.
- Conducts regular meetings with Team members with periodic site visits to provide supervision, ensures communication, and to monitor delivery of service.
- Assists with caseload coverage as needed including, but not limited to staff transitions and/or leave of absences.
Required Experience:
- Bachelor’s degree in a Social Work and/or the Human Services Field and at least one year working with adults diagnosed with developmental disabilities.
- Interpersonal skills to provide effective leadership for subordinate personnel, to maintain a variety of internal and external contracts, and to interact effectively with administrators and state officials.
- High level of initiative, self-direction, and accountability for actions required.
- Skill in promoting multidisciplinary collaboration; ability to act as a facilitator and change agent required.
- Analytical ability to manage a budget, as well as resolution of complex operational and sensitive interpersonal issues.
- Performance of principle duties and responsibilities require the ability to travel if needed.
- Must have and maintain a valid state driver’s license, maintain automobile insurance coverage and have access to an automobile.
Preferred Education and Experience
o Two to three years of supervisory experience.
o Support Coordinator Supervisor Experience Preferred.
